Care Coordinators are based in GP practices and are part of the NHS. 

 

This team works with people with multiple long term health conditions or older people who need extra support to live independently. 

 

To get a referral, contact your GP Practice. 

What do Care Coordinators do? 

Care Coordinators support residents with multiple long term health conditions or older people who need extra support to live independently.  

 

They can support you to: 

  • Prepare for your doctors and hospital appointments 
  • Develop a personalised care plan 
  • Follow up with you afterwards to see how the appointments went 
  • Connect you into other local services that could help you 

 

Care Coordinators offer home visits.
